Root cause

The underlying condition whose correction prevents the incident from recurring, as distinct from the symptom that was restored.

In REAP

REAP’s output is an evidence-backed root cause, with time to answer reported as P50 and P95 in a pilot; “restored” without a root cause is the “restored ≠ fixed” gap.
